24 Equipment Checkout Agreement

Students will be issued equipment to use for camps, rehearsals and practice at home.
Equipment will be released to students ONLY with parent/guardian permission.

Depending on availability or show requirements, not all students will be issued a rifle, sabre, and/or other show equipment (hoops, ribbons, swing flags, etc.)

Equipment Checkout Agreement

This portion lays out responsibilities of the students in order to keep equipment in good condition while also keeping up with required maintenance.

STUDENT: Please CHECK MARK the statements below to show you've read and understand them. I will NOT let anyone else touch, hold or spin my equipment. While on school grounds, I will NOT spin or do any other movements with my equipment unless it is during audition camp and/or rehearsals. I will always hold pole upright (in cradle), keep my rifle/sabre in my bag & no spinning when walking. I will always use my equipment outside, in an open area only, away from people, plants, trees & animals. I will only use the equipment to practice skills taught; unless otherwise directed (exceptions may be assignments to create 8-16 counts of choreography on your own). I will always roll up the silk when the pole is not in use. The silk is strong but can rip if caught on anything sharp. I will check my equipment for tape tears or tip/strap/bolt/hilt breakage/separation. I will use electric tape as needed or return to the equipment to have it evaluated, fixed or replaced.

I will report any issues with the equipment or my bag to my instructor immediately. I understand that the equipment is made out of aluminum (pole), wood (rifle), steel (sabre) and it may hurt, cause injury to others or myself self when used, either properly or improperly. I understand that accidents may happen. I will take breaks while practicing, massage/stretch hands, shoulders and arms as needed as well. I will stretch and ice/heat sore areas as needed. I understand that I must bring equipment back to OHHS for camp and/or rehearsals unless otherwise told. I understand that flag poles ARE NOT permitted on the school bus. I will have transportation available to bring my pole to OHHS when needed. I understand that if any of the equipment goes missing or becomes damaged beyond repair due to my own negligence, I may incur the repair/replacement charges on my school account to replace them.
Equipment Repair/Replacement Costs

All pieces of equipment are taped with student’s name and a number upon release. As long as students are diligent to bring attention to instructor when equipment looks like or is in need of repair, these costs will be waived. DO NOT ATTEMPT TO REPAIR equipment/bags on your own. Doing this may void any warranties we have and will require a fee for you to replace the item.

If we find equipment in the locker that is damaged and/or broken and there is no communication, that is when the following amounts are charged to the student's school account.

Missing equipment: We will notify students first and give them a chance to find and return their equipment. After 2 days, parents/guardians will be notified. When parents are contacted, an additional 2 days are given to return equipment before accounts are charged.

- Flagpole: $20 per pole, $20 per practice silk
- Rifle: $15 per strap, $15 per bolt, $65 per rifle
- Sabre: $35 per handle, $75 per sabre
- Hoop: $40 per hoop
- Ribbons: $15 per ribbon, $15 for rod/pole
- Bag: $40 per bag
